A complete application to the FIRST program consists of:

• A completed FIRST Application Form
• A copy of your curriculum vitae
• A completed copy of the FIRST Personal Data Form
• A letter from you that might include: why you are interested in FIRST; your experience/interest in teaching; your career plans after fellowship training
• Two letters of reference (under most circumstances one letter should be from your thesis advisor)
• A recent photograph of yourself (optional)
